Prescription Refills
For prescription refills, patients should contact their pharmacy first to request a refill or submit a request through the patient portal if a new prescription is needing to be sent. Please allow appropriate processing time for refill requests.

Referrals
If your insurance requires a referral, please contact our office with the specialist name, reason for referral, appointment date if scheduled, and fax number.
